Cardiology - About the Department

Right from the inception of Jaslok Hospital & Research Centre (JHRC) 42 years ago, the Cardiology Department has been a pioneer in its field and continues to be one of the leading centres for high-quality cardiac care, establishing Jaslok Hospital as one of the best heart hospitals in India.

It comprises various facilities ranging from preventive to diagnostic cardiology, paediatric to adult interventional cardiology and research projects to innovation. It ensures highest quality of care, while maintaining ethical and patient-centric practices.

Being a high-volume centre, the consulting doctors have experience in handling routine and complex cases, which either involves cardiac intervention or surgery. After the first scientific symposium and workshop on percutaneous transluminal coronary angioplasty in the year 1988, the first of its kinds in India, the Department continues to be a leader in innovation in the field of interventional cardiology.

JHRC has the distinction of being the first hospital in Mumbai to introduce 2D echocardiography. It is also the first to perform electrophysiology study (EPS), first to use an Impella left ventricular assist device, implantation of the first drug-eluting stent and also pioneering the first transcatheter aortic valve replacement (TAVR).

Interventional Services

JHRC specialises in detecting coronary artery blockages by using highly advanced treatment methods.

With all services and treatments under one roof, Jaslok Hospital offers CT coronary angiography; invasive coronary angiography and angioplasty; rotablation (a procedure where special diamond-tipped steel burrs are used to clean up calcium from coronary arteries); high-risk angioplasty with support to heart muscle using intra-aortic balloon pump (IABP) and latest coronary imaging techniques such as intravascular Ultrasound of the coronary arteries (IVUS) with virtual histology and optical coherence tomography (OCT). The Department has an excellent track record of opening up arteries that are as much as 100% occluded.

A number of workshops and live demonstrations have been organised and projected in real-time within the country as well as abroad.

Primary Angioplasty in Acute Myocardial Infarction

JHRC is also a leader in angioplasty in India. The Department has pioneered the PAMI PROJECT, which has been specifically designed for patients who have had major heart attacks and for whom timely medical intervention is critical. The cardiac team is available around the clock, which includes a senior and a junior cardiologist, a cardiac anaesthetist, technician and a nurse, in addition to the emergency services staff.

When a patient with a major heart attack is brought to the hospital, the PAMI code is activated and the patient is immediately transferred to the catheterisation lab and the totally occluded artery is opened with a stent. The entire process is performed within 60 to 90 minutes, making it the best possible and fastest treatment for heart attacks.

The staff is promptly available and adequately trained to deal with all adverse scenarios. We are proud to conform to international standards because we believe that our patients deserve the best.

Non-invasive Services

It includes:

o 2D echocardiography with speckle tracking
o 2D and 3D strain rate imaging
o Dobutamine stress echocardiography
o Transoesophageal echocardiogram
o 24 hours non-invasive BP monitoring
o 24-hour and 48-hour Holter monitoring
o Radio-nuclear scans: single photon emission computed tomography (SPECT) and positron emission tomography (PET)
o Cardiac CT Scan and MRI

This line of treatment completely evaluates the heart condition non-invasively at a competitive price. The best services conducted by trained cardiologists are used to diagnose and monitor heart patients. We take pride in ourselves for providing long-term follow-up for our patients.

Arrhythmias

Many patients suffer from a condition in which their heart may either beat too fast or too slow. A comprehensive testing is done to diagnose patients with rhythm disturbances, such as 24-hour and 48-hour Holter monitoring, loop recording, tilt-table testing and EPS.

Patients with slow heartbeats require pacemakers. The Department has some of the best Electrocardiologists who implant single-chamber or dual-chamber pacemakers. At this centre, cardiac resynchronization therapy (CRT-P) is performed, where a specialized pacemaker is implanted to improve the heart pumping function and treat heart failure. In special situations, CRT-D or a or combo device is used to treat heart failure and ventricular arrhythmias.

For patients whose hearts beat too fast, several treatment options including defibrillation and cardioversion, radiofrequency ablation and implantation of automated implantable cardioverter-defibrillator (AICD) are available.

Heart Failure and Cardiac Rehabilitation

Heart failure implies the inability of the heart to adequately pump blood. Jaslok has specialised heart failure clinics where patients are evaluated by a cardiologist and cardiothoracic surgeon. Integrated care is provided to patients with focus on optimising medical management and rehabilitation with the help of a physiotherapist and psychiatrist.

Comprehensive advice is provided regarding the treatment options available, such as revascularisation by angioplasty or heart surgery, cardiac resynchronisation therapy, intra-aortic balloon pump and left ventricular assist devices. The hospital has also launched a heart transplant unit where senior cardiothoracic surgeons trained in cardiac transplantation are on board.

Patients and their families are advised and trained to cope with this debilitating disease, and are offered the best care to restore their health.

Valvular Heart Disease and Cardiomyopathy

Quality care is provided for a wide spectrum of cardiac diseases. Ailments such as rheumatic, mitral and aortic valve disease; nonrheumatic valvular regurgitation or stenosis; dilated cardiomyopathy; hypertrophic cardiomyopathy and many other rare cardiac diseases are evaluated and a long-term treatment plan is provided.

Patients with mitral valve stenosis are treated with balloon mitral valvuloplasty where the valve can be opened non-surgically using a balloon. Valve replacement surgeries are performed by the best cardiothoracic surgeons with excellent results.

We are proud to be one of the first few centres in India to have performed a procedure called transcatheter aortic valve implantation (TAVI) for patients with aortic stenosis, using only the groin arteries. This state-of-the-art technology is a comfortable and convenient way to cure a valve disease without any surgical incisions. This is the only centre in Mumbai to have performed TAVI.

Paediatric Cardiology

With ultramodern echocardiography equipment for evaluating heart disease in children, we offer correction of defects such as a hole in the heart and narrowing of valves from birth. State-of-the-art treatment is provided for many other congenital heart defects, including atrial septal defect, ventricular septal defect and patent ductus arteriosus device closure.

"I am a Full Time Consultant Cardiologist working at Jaslok Hospital and Research Centre since the last 6 years. I have received special training in Echocardiography and Interventional Cardiology as well as Cardiovascular Imaging.

I have a special interest in Cardiovascular Research and am currently involved in several research projects conducted by Jaslok Hospital.

I am also a Member of the Apex Research Committee and Scientific Committee of Jaslok Hospital. I was also a part of the Core committee for formulation of the Indian Guidelines on Hypertension III – 2013. I am the editor of several books for medical students and practitioners.
