o Discharge of the patient will be authorized as per the doctor's opinion and written order only.

o In case the patient/relative wishes to be transferred to another hospital or be sent home against the advice of the doctor, the patient/relative would be required to sign a 'Discharge Against Medical Advice' declaration and the hospital will not be held responsible for any change in the patient's condition, thereafter.

o When your doctor feels that you are able to go home he/she will inform you and indicate this in your medical records.

o The Ward clerk will inform you when your final bill is ready. You will have to go to in-patient billing department located in podium to settle the bill. The billing department will give you a bill clearance receipt on your settling the bill.

o Daily discharged patients are personally seen by the CEO twice a day (11.00 a.m. and 3.00 p.m.) for their feedback and suggestions.

o A copy of the bill clearance receipt has to be submitted to the Wards clerk who will then give you your discharge summary and investigation report. The management will try their best to complete your discharge formalities as soon as possible, however, at an average the discharge process takes 2 to 3 hours. The discharge time may be more for credit/TPA patients, subject to approvals by TPA/Credit Company. The inconvenience cause is deeply regretted, so please plan your departure accordingly.

o In case, you require wheel chair/stretcher/ambulance service, please inform the ward clerk for the same.

o Please vacate the room at the earliest, after discharge as the other patients will require the facilities.

o Kindly ensure to return the attendant's and visitor's passes to the billing department after your discharge.
